Transition Assessments and Practices

Audience:  This workshop is intended for classroom teachers, all related service providers (OT, PT, SLP, LMHP, etc.), special educators, parents, community providers, agency staff, and paraeducators who work with students (ages 14-21) with disabilities. 

Description:  
Day 1:  Transition Assessment for All Learners. Let’s dig in and learn about transition assessment in the morning then apply what we’ve learned to case studies in the afternoon. Pick a work group and get ready to learn together. 

Day 2:  Developing Meaningful Transition Activities- Traveling Towards Successful Post-School Outcomes. Morning content will focus on students who are participating in grade level curriculum, and the afternoon content will center around developing activities for students with significant support needs.
 
During these sessions, we will build upon strengths, preferences, interests, and needs (SPIN), and select meaningful transition activities and leave with resources for completing those activities. You will develop a transition activity that aligns with each post-secondary goal and select the materials and resources to complete the activity. 

Day 3:  Developing a Plan- Using CAPS to Build a Meaningful Roadmap. Let’s build a plan that works with the tools to make it happen. We will integrate work experiences, agency connections, academic and behavioral strategies into the roadmap, focusing on effective practices in service delivery.
